Tetrabenazine is a generic prescription medication sold under the brand name Xenazine in the United States and Nitoman in Canada. It belongs to a class of medications called vesicular monoamine transporter 2 (VMAT2) inhibitors.
Tetrabenazine is FDA-approved for the treatment of chorea associated with Huntington’s disease — a hereditary, progressive neurological condition in which nerve cells in the brain gradually break down, causing a person to lose control of movements, thoughts, and emotions. By inhibiting VMAT2, tetrabenazine reduces the release of dopamine and other monoamine neurotransmitters that trigger involuntary, dance-like movements.
Tetrabenazine is also used off-label for tardive dyskinesia, a movement disorder that may develop as a side effect of long-term use of certain psychiatric medications.
FDA Boxed Warning: Tetrabenazine can increase the risk of depression and suicidal thoughts and behavior in patients with Huntington’s disease. Patients, caregivers, and families should monitor closely for mood changes and contact their physician immediately if concerns arise. Tetrabenazine is contraindicated in patients with active suicidality or untreated depression.

If you miss a dose, take it as soon as you remember. However, if it is close to the time of your next scheduled dose, skip the missed dose and continue your regular schedule. Do not double up on doses.
If you miss doses for more than 5 consecutive days, consult your doctor before restarting. Retitration (starting over at a lower dose) is typically recommended after an interruption of more than 5 days.
Tetrabenazine may be discontinued without tapering. However, chorea symptoms may return within 12 to 18 hours after the last dose as the medication clears your system. Always consult your physician before making any changes to your medication regimen.
