Tetrabenazine is a generic prescription medication sold under the brand name Xenazine in the United States and Nitoman in Canada. It belongs to a class of medications called vesicular monoamine transporter 2 (VMAT2) inhibitors.

Tetrabenazine is FDA-approved for the treatment of chorea associated with Huntington’s disease — a hereditary, progressive neurological condition in which nerve cells in the brain gradually break down, causing a person to lose control of movements, thoughts, and emotions. By inhibiting VMAT2, tetrabenazine reduces the release of dopamine and other monoamine neurotransmitters that trigger involuntary, dance-like movements.

Tetrabenazine is also used off-label for tardive dyskinesia, a movement disorder that may develop as a side effect of long-term use of certain psychiatric medications.

Tetrabenazine (Xenazine) is FDA-approved to treat chorea associated with Huntington’s disease in the United States. Chorea refers to the involuntary, irregular movements that are one of the hallmark symptoms of Huntington’s disease.

Tetrabenazine is also used off-label for tardive dyskinesia, a movement disorder caused by long-term use of certain psychiatric medications. In Canada, it is additionally approved for chronic tic disorders including Tourette syndrome.

Tetrabenazine is available in two tablet strengths:

  • 12.5 mg tablets
  • 25 mg tablets

Tetrabenazine is taken orally with or without food. Dosing is individualized and adjusted gradually under a physician’s supervision:

  • Typical starting dose: 12.5 mg once daily in the morning
  • May be increased to 12.5 mg twice daily after one week
  • Further increases of 12.5 mg may be made at weekly intervals up to 50 mg/day
  • Doses greater than 37.5 mg/day are divided into three daily doses, with no single dose exceeding 25 mg

Always follow your physician’s instructions exactly. Never adjust your dose without first consulting your doctor.

Common side effects of tetrabenazine (reported in more than 10% of patients) include:

  • Drowsiness and sedation
  • Fatigue and loss of energy
  • Insomnia
  • Nausea
  • Akathisia (restlessness or an uncontrollable urge to move)
  • Depression and anxiety
  • Extrapyramidal symptoms such as parkinsonism (slow movement, muscle stiffness)
  • Falling
  • Upper respiratory tract infections

Important: Tetrabenazine carries an FDA Boxed Warning for an increased risk of depression and suicidal thoughts and behavior in patients with Huntington’s disease. Contact your physician immediately if you experience mood changes, worsening depression, or thoughts of self-harm.

Yes. Tetrabenazine is contraindicated (should not be used) in patients who:

  • Are actively suicidal
  • Have untreated or inadequately treated depression
  • Have hepatic (liver) impairment
  • Are currently taking monoamine oxidase inhibitors (MAOIs) or have taken them within the past 2 weeks
  • Are currently taking reserpine (at least 20 days should pass after stopping reserpine before starting tetrabenazine)
  • Are currently taking deutetrabenazine or valbenazine

Always inform your doctor about all medications, supplements, and medical conditions before starting tetrabenazine.

If you miss a dose, take it as soon as you remember. However, if it is close to the time of your next scheduled dose, skip the missed dose and continue your regular schedule. Do not double up on doses.

If you miss doses for more than 5 consecutive days, consult your doctor before restarting. Retitration (starting over at a lower dose) is typically recommended after an interruption of more than 5 days.

Tetrabenazine may be discontinued without tapering. However, chorea symptoms may return within 12 to 18 hours after the last dose as the medication clears your system. Always consult your physician before making any changes to your medication regimen.
