What is erlotinib (Tarceva) used to treat?

Erlotinib is FDA-approved to treat certain cancers as a targeted EGFR tyrosine kinase inhibitor.

FDA-approved (labeled) indications:

  • Metastatic non–small cell lung cancer (NSCLC): in tumors with EGFR exon 19 deletions or exon 21 (L858R) substitution mutations, as detected by an approved test — as first-line therapy, maintenance treatment, or as second-or-greater-line treatment after progression following at least one prior chemotherapy regimen
  • Locally advanced, unresectable, or metastatic pancreatic cancer: as first-line treatment, in combination with gemcitabine

Important limitations of use:

  • Use in combination with platinum-based chemotherapy is not recommended
  • Safety and efficacy for metastatic NSCLC with EGFR mutations other than exon 19 deletion or exon 21 (L858R) substitution have not been established

How does erlotinib work?

Erlotinib is an epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor. It reversibly inhibits the tyrosine kinase activity of the EGFR (also called HER1/EGFR). By blocking intracellular phosphorylation, erlotinib prevents downstream signaling that cancer cells rely on to grow and survive, which results in cancer cell death.

Erlotinib has a higher binding affinity for EGFR exon 19 deletion or exon 21 (L858R) mutations than for the normal (wild-type) receptor, which is why EGFR mutation testing is used to identify patients most likely to benefit.

How do I take erlotinib?

Erlotinib is taken by mouth once daily on an empty stomach — at least 1 hour before or 2 hours after eating. Take it at about the same time each day, exactly as prescribed by your doctor.

Typical dosing by indication (as prescribed):

  • Non–small cell lung cancer: 150 mg once daily, until disease progression or unacceptable toxicity
  • Pancreatic cancer: 100 mg once daily, in combination with gemcitabine

Important administration notes:

  • Empty stomach: food significantly increases how much erlotinib your body absorbs, so it should be taken without food as directed
  • Acid-reducing medications: proton pump inhibitors (PPIs) should be avoided if possible. If you take an H2-blocker, erlotinib should be dosed 10 hours after and at least 2 hours before the H2-blocker dose. Separate antacids by several hours.
  • Do not adjust your own dose — only your prescriber should change it

What are the common side effects of erlotinib?

The most common side effects of erlotinib involve the skin and digestive system. Many people have manageable side effects, but it is important to recognize potential problems early and report them to your healthcare team.

Common side effects (reported in more than 10% of patients):

  • Skin rash (very common; an acne-like rash may appear on the face, back, and chest), dry skin, itching, and nail changes (paronychia)
  • Diarrhea, decreased appetite, nausea, vomiting, and mouth sores (stomatitis)
  • Fatigue and weakness
  • Cough and shortness of breath
  • Eye effects such as conjunctivitis and dry eye
  • Back pain, joint pain, and muscle/skeletal pain
  • Chest pain

Serious side effects — get medical help right away:

  • Lung/breathing problems: rare but sometimes serious interstitial lung disease (shortness of breath, cough, fever)
  • Liver problems: dark urine, yellowing of skin or eyes, severe fatigue, stomach pain
  • Severe diarrhea or dehydration
  • Severe skin reactions: blistering, peeling, or a reaction suggestive of Stevens-Johnson syndrome
  • Eye problems: eye pain, severe irritation, or changes in vision
  • Stomach/intestinal perforation, or signs of bleeding (vomiting blood; black, tarry stools)
  • Stroke or heart symptoms: chest pain or pressure, weakness on one side, trouble speaking

Can I take other medications, foods, or supplements while on erlotinib?

Erlotinib interacts with many medications and some foods, so tell your doctor and pharmacist about everything you take — including prescription drugs, over-the-counter products, vitamins, and supplements. Erlotinib is processed by the CYP3A4 and CYP1A2 enzyme systems, which is the basis for many of these interactions.

Important interactions to be aware of:

  • Grapefruit and grapefruit juice: avoid entirely — they can increase erlotinib levels in your body
  • Acid-reducing medications: proton pump inhibitors should be avoided if possible; H2-blockers and antacids require careful dose separation (see “How do I take erlotinib?”)
  • Certain antibiotics and antifungals (such as ciprofloxacin and strong CYP3A4 inhibitors) can raise erlotinib levels
  • Certain seizure medications and other CYP3A4/CYP1A2 inducers can lower erlotinib levels
  • Warfarin: erlotinib may increase its blood-thinning effect; extra INR monitoring may be needed
  • Smoking: tobacco smoke can significantly lower erlotinib levels — tell your doctor if you smoke

Does smoking affect erlotinib?

Yes. Cigarette smoking can substantially lower the amount of erlotinib in your body, which may make it less effective. For this reason, patients are advised to stop smoking if possible.

If smoking cannot be avoided, the prescriber may adjust the erlotinib dose under careful monitoring — and the dose must be reduced back to the standard starting dose promptly once smoking stops. Never adjust your dose on your own; this should only be done by your prescriber.

What monitoring or lab tests are needed while taking erlotinib?

Your healthcare provider will monitor you regularly while you take erlotinib to check for side effects and ensure the medication is working safely.

Common monitoring includes:

  • Liver function tests (transaminases, bilirubin, alkaline phosphatase) — periodically, and more frequently if you have liver impairment
  • Kidney function tests and electrolytes — especially if you are at risk for dehydration
  • INR / prothrombin time — if you also take warfarin
  • EGFR mutation status — for patients with NSCLC, to confirm erlotinib is appropriate
  • Eye (ophthalmologic) assessment — a baseline exam may be considered, with reassessment if eye symptoms develop
  • Hepatitis B screening — recommended before starting systemic anticancer therapy

Can I take erlotinib if I’m pregnant or breastfeeding?

Erlotinib may cause harm to an unborn baby and is generally not recommended during pregnancy. Discuss family planning with your healthcare team before starting treatment.

Important information:

  • Contraception: patients who could become pregnant should use effective contraception during treatment and for at least 1 month after the last erlotinib dose
  • Pregnancy: based on its mechanism and animal data, erlotinib may cause fetal harm; erlotinib is known to cross the placenta
  • Breastfeeding: it is not known whether erlotinib passes into breast milk. Because of the potential for serious side effects in a breastfed infant, patients should not breastfeed during treatment and for 2 weeks after the final dose

Always discuss pregnancy, contraception, and breastfeeding with your oncologist before and during erlotinib treatment.

How should I store erlotinib?

Store erlotinib at room temperature, 77°F (25°C), with brief excursions permitted between 59°F and 86°F (15°C to 30°C). Keep it in its original container, away from excess heat and moisture, and out of reach of children and pets.
