Federal guidance from the HHS Office of Inspector General explains that manufacturer copay coupons cannot be used by patients whose drug is paid for by federal health care programs such as Medicare or Medicaid.

Lexidrug lists an Average Wholesale Price (AWP) of $337.50 per Prevymis tablet for both the 240 mg and 480 mg strengths. At one tablet a day, 30 days works out to about $10,125 at AWP. Oral pellet packets are listed at $14.05 (20 mg) and $84.31 (120 mg) each.*

Prevymis (letermovir) is an antiviral medicine, first approved by the FDA on November 8, 2017. It is used to prevent cytomegalovirus (CMV) infection and disease in two groups of transplant patients:
Stem cell (hematopoietic cell) transplant: adults and children 6 months and older weighing at least 6 kg who are CMV-seropositive recipients of an allogeneic transplant.
Kidney transplant: high-risk adults and children 12 years and older weighing at least 40 kg, where the donor is CMV-positive and the recipient is CMV-negative (D+/R-).

For adults, the usual dose is 480 mg once a day, by mouth or by IV. After a stem cell transplant, it is usually started between day 0 and day 28 and continued through day 100; some patients may continue through day 200. After a high-risk kidney transplant, it is usually started between day 0 and day 7 and continued through day 200. Your transplant team may lower the dose if you also take cyclosporine.
Tablets: take with or without food and swallow whole. Oral pellets: do not crush or chew; mix with 1 to 3 teaspoons of soft food (such as applesauce or yogurt) that is at or below room temperature, and take within 10 minutes. IV: used only for patients who cannot take it by mouth, and switched to oral as soon as possible.

Kidney: no dose change is needed when CrCl is above 10 mL/minute. The labeling gives no dose advice for CrCl of 10 or less or for dialysis, though Lexidrug notes a change is unlikely because very little leaves the body in urine. For the IV form, kidney function is watched closely when CrCl is below 50 mL/minute, because an ingredient in the IV solution can build up.
Liver: no change for mild or moderate liver problems (Child-Pugh A or B). Use is not recommended with severe liver problems (Child-Pugh C).

In adults, side effects reported in more than 10% of patients include nausea (27%), diarrhea (26%), vomiting (19%), swelling in the legs or arms (14%), cough (14%), headache (14%), tiredness (13%), heart rhythm changes (13%, including atrial fibrillation 3% and fast heartbeat 4%), stomach pain (12%) and low platelet counts.
Call your doctor right away for a fast or abnormal heartbeat, swelling, changes in how much you urinate, signs of infection, unexplained bruising or bleeding, or signs of an allergic reaction. With the IV form, also report ringing in the ears or hearing changes.

Other viruses: Prevymis only works against CMV. Your team may add another antiviral (such as acyclovir) to protect against herpes simplex virus.
Resistance: CMV can become resistant to letermovir through changes in a viral gene called UL56.
IV form: contains hydroxypropyl betadex; it is meant for patients who cannot take pills and, if possible, should not be used for more than 4 weeks.
Older adults: in clinical trials, 15% of patients were 65 or older, and no differences in safety or effectiveness were seen compared with younger patients.

Yes, many. Prevymis must not be taken with pimozide or ergot medicines, and must not be combined with pitavastatin or simvastatin when you also take cyclosporine.
It can raise levels of several transplant medicines, including cyclosporine and sirolimus, and your team may monitor tacrolimus levels. It can lower levels of voriconazole and may affect warfarin. Atorvastatin is limited to 20 mg a day with Prevymis, and is not recommended at all when cyclosporine is also used. Carbamazepine should be avoided.

Your transplant team will check for CMV coming back (reactivation) and will check your kidney function (serum creatinine), especially if you get the IV form and your CrCl is below 50 mL/minute.

Some animal studies showed harmful effects, and it is not known whether letermovir passes into breast milk. Talk with your transplant team about the risks and benefits if you are pregnant, planning a pregnancy or breastfeeding.

Yes, as an off-label use. Lexidrug lists letermovir as an alternative for CMV prevention in at-risk liver, lung, heart and kidney transplant recipients, supported by the Fourth International Consensus Guidelines on CMV in Solid Organ Transplantation (2025), mainly for patients who cannot tolerate other antivirals such as valganciclovir.
Off-label means the use is not approved by the FDA for that condition. Insurance coverage and assistance eligibility for off-label use may differ from approved uses.
