Ofev (nintedanib) is an oral prescription drug. The FDA approved it for three lung conditions in adults.

FDA-Approved Indications for Ofev:

  • Idiopathic pulmonary fibrosis (IPF): A chronic lung disease that scars lung tissue. The cause is unknown. Ofev slows the decline of lung function in adults with IPF.
  • Progressive pulmonary fibrosis (PPF): Ofev treats chronic progressive fibrosing interstitial lung diseases (ILDs). These ILDs keep getting worse despite treatment. Causes include rheumatoid arthritis, sarcoidosis, and hypersensitivity pneumonitis.
  • Systemic sclerosis–associated interstitial lung disease (SSc-ILD): Ofev slows the decline of lung function in patients with ILD from systemic sclerosis (scleroderma).

Ofev does not cure these conditions. It does not reverse existing scar tissue. It works by slowing disease progression and protecting lung function.

You take Ofev (nintedanib) by mouth as a capsule. Take it twice a day, about 12 hours apart.

Ofev Dosing Information:

  • Standard dose (no liver impairment): 150 mg every 12 hours with food. Maximum dose: 300 mg per day.
  • Reduced dose (mild hepatic impairment, Child-Pugh A): 100 mg every 12 hours with food.
  • Moderate to severe hepatic impairment: Ofev is not recommended.
  • If you miss a dose, skip it. Take the next dose at the regular time. Do not double up.

Important Administration Notes:

  • Swallow capsules whole with liquid. Do not open, chew, or crush. The contents taste bitter and can irritate.
  • If you touch the capsule contents, wash your hands right away.
  • If you smoke, stop before starting Ofev. Smoking lowers Ofev exposure by about 21%.

Diarrhea is the most common side effect of Ofev. Most patients get it. GI side effects are usually mild to moderate. They happen most often in the first 3 months of treatment.

Common Ofev Side Effects (>10%):

  • Diarrhea (62% to 76% of patients) — the most frequent side effect
  • Nausea (24% to 32%)
  • Vomiting (12% to 25%)
  • Abdominal pain (15% to 18%)
  • Decreased appetite (9% to 11%)
  • Weight loss (10% to 12%)
  • Increased liver enzymes (13% to 14%)
  • Fatigue (10% to 11%)
  • Nasopharyngitis (13%)

Many patients keep loperamide (Imodium) on hand for diarrhea. Drink plenty of water. Talk to your healthcare provider about any side effect that worries you.

Yes. Ofev has important warnings. Discuss them with your prescriber before and during treatment.

Important Ofev Warnings:

  • Liver toxicity: Serious liver injury can happen. Some cases have been fatal. You will need liver function tests (ALT, AST, bilirubin) before you start Ofev and during the first 3 months. Risk is higher in women, patients under 65 kg, Asian patients, and older patients.
  • Diarrhea: Severe diarrhea can cause life-threatening dehydration. Call your doctor right away if it is severe or won’t stop.
  • Bleeding: Ofev can raise the risk of serious bleeding. Use with care if you have a bleeding risk or take anticoagulants.
  • Arterial thromboembolic events: Heart attacks can happen. Use with care if you have cardiovascular risk factors.
  • GI perforation: Rare cases of gastrointestinal perforation can happen. Some have been fatal. Use with care after abdominal surgery, with diverticular disease, or with NSAIDs or corticosteroids.
  • Fetal harm: Ofev can cause birth defects or fetal death. Patients who can get pregnant must use highly effective birth control during treatment and for 3 months after the last dose.

  • Keep at 68°F to 77°F (20°C to 25°C)
  • Brief shifts between 59°F and 86°F (15°C and 30°C) are okay

IPF is a chronic lung disease. Scar tissue (fibrosis) builds up in the lungs. The cause is unknown. “Idiopathic” means the origin is unknown.

As lungs scar, they get stiffer. They cannot move oxygen into the bloodstream as well. Patients feel more breathless over time.

  • IPF mostly affects adults over 60
  • It is more common in men and people with a smoking history
  • There is no cure for IPF. Treatment focuses on slowing the disease
  • Ofev (nintedanib) and pirfenidone are the two FDA-approved drugs that slow lung function decline in IPF
  • Symptoms include shortness of breath, dry cough, fatigue, and a crackling sound in the lungs when you breathe

If you have IPF, work with a pulmonologist who specializes in interstitial lung diseases.

PPF describes a pattern of disease behavior. It is not one specific condition.

In PPF, scarring keeps getting worse despite treatment. This happens in chronic fibrosing interstitial lung diseases (ILDs) no matter the underlying cause.

Unlike IPF, PPF has a known cause. It can show up with:

  • Rheumatoid arthritis
  • Sarcoidosis
  • Hypersensitivity pneumonitis
  • Mixed connective tissue disease
  • Other inflammatory conditions

When these ILDs keep getting worse and don’t respond to standard treatment, Ofev slows further lung function decline.

A pulmonologist manages PPF. They often work with a rheumatologist or another specialist.

SSc-ILD is a lung complication of systemic sclerosis (scleroderma). Systemic sclerosis is a rare autoimmune disease. It hardens the skin and connective tissues.

When systemic sclerosis hits the lungs, it scars and inflames lung tissue (ILD). This makes breathing harder over time.

SSc-ILD is one of the top causes of illness and death in systemic sclerosis patients. Ofev slows the decline of lung function in SSc-ILD. A rheumatologist and pulmonologist both manage this condition.
