Brukinsa (zanubrutinib) is a brand-name oral cancer medicine used to treat chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma, Waldenström macroglobulinemia and certain other lymphomas in adults. There is no generic in the United States, so the out-of-pocket cost can be hard to manage.

Who is eligible for Brukinsa copay assistance?

Each program sets its own rules. Manufacturer copay cards are generally limited to patients with commercial or private insurance. Federal guidance from the HHS Office of Inspector General explains that manufacturer copay coupons cannot be used by patients whose drug is paid for by federal health care programs such as Medicare or Medicaid.

Patients with Medicare, Medicaid or no insurance may still have options, such as the manufacturer’s patient assistance program or an independent foundation grant for their condition. These usually have their own requirements, such as income and insurance documents.

Is there a copay card for Brukinsa?

BeOne Medicines, the maker of Brukinsa, runs a program called myBeOne Support. According to BeOne, eligible patients with commercial insurance may pay $0 per prescription, up to an annual maximum that BeOne sets.*

Manufacturer copay coupons generally cannot be used by patients whose prescription is paid for by a federal health care program such as Medicare or Medicaid. For patients with Medicare, VA, DoD or TRICARE, BeOne says it provides information on independent charitable organizations.

No. Lexidrug lists no generic zanubrutinib available in the United States as of September 2026. Brukinsa is the only US brand.

What is Brukinsa (zanubrutinib) used for?

Brukinsa (zanubrutinib) is an oral cancer medicine called a Bruton tyrosine kinase (BTK) inhibitor. It was first approved by the FDA on November 14, 2019. In adults, it is approved to treat:

Chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL); Waldenström macroglobulinemia; mantle cell lymphoma after at least 1 prior therapy; relapsed or refractory marginal zone lymphoma after at least 1 anti-CD20-based treatment; and relapsed or refractory follicular lymphoma after 2 or more treatments, taken together with obinutuzumab.

How is Brukinsa taken?

The usual adult dose is 160 mg twice a day or 320 mg once a day, taken until the disease progresses or side effects become too hard to manage. It can be taken with or without food.

Capsules (80 mg): swallow whole with water; do not open, break or chew. Tablets (160 mg): swallow whole with water; do not chew or crush; the tablet may be split in half. If you miss a dose, take it as soon as you can the same day, then go back to your normal schedule the next day. Avoid grapefruit, grapefruit juice and Seville oranges.

Are dose changes needed for kidney or liver problems?

Kidney: no dose change is needed when CrCl is 15 mL/minute or higher. The labeling has not studied CrCl below 15 or dialysis, though Lexidrug notes a change is unlikely because very little leaves the body in urine.

Liver: no change for mild or moderate liver problems (Child-Pugh A or B). For severe liver problems (Child-Pugh C), the dose is reduced to 80 mg twice a day. If liver injury from the drug is suspected, it is held, and it is stopped if confirmed.

What are the common side effects of Brukinsa?

In adults, common side effects include upper respiratory infections (27% to 44%), muscle and bone pain (26% to 45%), bleeding of any kind (24% to 42%), rash (20% to 29%), bruising (16% to 24%), tiredness (13% to 31%), diarrhea (14% to 22%), high blood pressure (14% to 19%), pneumonia (12% to 18%) and cough (11% to 16%). Lab changes such as low white blood cells, low platelets and high blood sugar are also common.

Call your doctor right away for signs of infection, bleeding you cannot stop, black or bloody stools, chest pain or an irregular heartbeat, fainting, yellow skin or eyes, or signs of stroke.

What serious warnings should I know about?

Bleeding: serious and fatal bleeding has occurred; the risk may be higher with blood thinners or antiplatelet medicines.

Infections: serious infections, including pneumonia and hepatitis B reactivation, have occurred.

Heart rhythm problems: atrial fibrillation, atrial flutter and ventricular arrhythmias have occurred.

Low blood counts, liver injury and second cancers: severe low blood counts, serious liver injury and new cancers (most often non-melanoma skin cancer) have been reported. Use sun protection.

Surgery: your team may pause Brukinsa for 3 to 7 days before and after surgery, depending on bleeding risk.

Can Brukinsa interact with other medications?

Yes. Strong CYP3A4 inducers should be avoided because they lower Brukinsa levels. Strong and moderate CYP3A4 inhibitors raise Brukinsa levels, so the dose is lowered (to 80 mg once a day or 80 mg twice a day). Blood thinners and antiplatelet medicines may raise the risk of bleeding. Live vaccines should be avoided.

What monitoring is needed on Brukinsa?

Your care team will check your blood counts regularly and your liver tests at the start and during treatment. They will watch for bleeding, infection and heart rhythm problems, check for new skin or other cancers, and may screen for hepatitis B before you start.

Can I take Brukinsa during pregnancy or breastfeeding?

Brukinsa may harm an unborn baby. Pregnancy should be checked before starting. Patients who could become pregnant, and patients whose partners could become pregnant, should use effective birth control during treatment and for 1 week after the last dose. Breastfeeding is not recommended during treatment and for 2 weeks after the last dose.

Is Brukinsa safe for older adults?

In clinical trials, about half of patients were 65 or older, and no major differences in effectiveness were seen. Patients 65 and older did have more serious side effects than younger patients, so your team may watch you more closely.

How should Brukinsa be stored?

Store capsules and tablets at room temperature, 20°C to 25°C (68°F to 77°F). Brief exposure to 15°C to 30°C (59°F to 86°F) is allowed.
