Adempas (riociguat) lists at $197.42 per tablet, or about $17,768 for a 30-day supply at the usual 3 tablets a day, and generic riociguat lists at $177.68 per tablet, about $15,991 a month, based on Average Wholesale Price from Medi-Span via Lexidrug (October 10, 2026). Those are list prices before insurance. What you actually pay depends on your plan and on the assistance you qualify for, and the right assistance can bring it down.
This guide breaks down what Adempas costs, why the monthly total climbs so fast, how brand and generic compare, and the real ways to pay less.
How Much Does Adempas Cost Without Insurance?
Without insurance, a month of Adempas lists at about $17,768, and a month of generic riociguat at about $15,991.
Every strength of Adempas (0.5 mg, 1 mg, 1.5 mg, 2 mg and 2.5 mg) carries the same per-tablet list price, and so does every strength of generic riociguat, according to Lexidrug. That means a dose increase during titration does not change the price per tablet.
Why Does the Monthly Cost of Adempas Add Up So Fast?
Adempas is taken 3 times a day, so a single month means about 90 tablets.
Adempas is a prescription tablet for adults with pulmonary arterial hypertension (PAH, WHO Group 1) and for chronic thromboembolic pulmonary hypertension (CTEPH, WHO Group 4) that is inoperable or that returns after surgery (FDA prescribing information). Lexidrug lists dosing of 3 times a day, starting at 1 mg (or 0.5 mg in patients at higher risk of low blood pressure) and increasing by 0.5 mg no sooner than every 2 weeks, up to 2.5 mg 3 times a day. At about $197 per tablet, 3 tablets a day is close to $600 a day at list price.
Always take riociguat exactly as prescribed. Do not change your dose or how often you take it without talking to your prescriber.
Is Generic Riociguat Cheaper Than Adempas?
Yes, at list price generic riociguat costs about 10% less than brand Adempas: $19.74 less per tablet, or about $1,777 less for a 30-day supply.
Generic riociguat is available in the United States in the same 5 strengths as the brand (Lexidrug). Whether the generic actually costs you less depends on your plan, because insurers set their own prices and preferred drugs. What Does Adempas Cost With Insurance?
With insurance, your cost depends on your plan’s deductible, copay or coinsurance, and whether the plan covers the brand, the generic, or both.
Commercial insurance: check your plan’s formulary to see how riociguat is covered and whether prior authorization is needed. Manufacturer copay programs are generally available only to commercially insured patients, and eligibility is set by the sponsor.
Medicare: Adempas and generic riociguat are pharmacy-filled tablets, so they fall under Medicare Part D. Part D has a yearly out-of-pocket limit for covered drugs ($2,100 in 2026), and the Medicare Prescription Payment Plan lets you spread those costs into monthly payments. People with limited income may qualify for Extra Help. Details are on Medicare.gov. Manufacturer copay cards cannot be used with Medicare, Medicaid or other government insurance.
How Can You Lower Your Adempas Cost?
The most common ways to lower your Adempas cost are manufacturer copay programs, patient assistance programs, and independent foundation grants.
- Manufacturer copay programs: for patients with commercial insurance; they can reduce your copay for the brand.
- Patient assistance programs: for patients with limited or no insurance, usually based on household income.
- Foundation grants: independent charities that may help Medicare and other patients with pulmonary hypertension pay their share when a fund is open.
- Ask about the generic: talk to your prescriber and your plan about whether generic riociguat would cost you less.

“Most families see the list price of Adempas and assume they can’t afford it. Once we look at their insurance and every option they qualify for, the number they actually pay can look very different. Our job is to do that legwork for them.
