What is Noxafil (posaconazole) used to treat?
Noxafil (posaconazole) is FDA-approved to treat invasive aspergillosis in adults and pediatric patients 2 years and older, oropharyngeal candidiasis in adults and pediatric patients 13 years and older (including patients refractory to itraconazole or fluconazole), and to prevent invasive Aspergillus and Candida infections in severely immunocompromised patients—including those receiving hematopoietic stem cell transplants with graft-versus-host disease, or patients with hematologic malignancies and prolonged neutropenia due to chemotherapy. Posaconazole works by interfering with fungal cell membrane formation, inhibiting fungal growth.
What’s the difference between Noxafil tablets, IV, and oral suspension?
Noxafil (posaconazole) is available in four formulations that are NOT interchangeable due to dosing differences:

  • Delayed-release tablets (100 mg): The most commonly prescribed form for adults and pediatric patients weighing more than 40 kg. Preferred over the immediate-release suspension because it is easier to administer, better tolerated, and more reliably absorbed.
  • Intravenous (IV) solution (300 mg/16.7 mL): Used in hospitalized patients or those unable to tolerate oral therapy. Administered via central line.
  • Delayed-release oral suspension: For pediatric patients 2 years and older weighing 10 to 40 kg. Reconstituted just before administration.
  • Immediate-release oral suspension (40 mg/mL): Must be taken with a full meal or acidic carbonated beverage for proper absorption.

Your prescriber will choose the formulation based on your age, weight, diagnosis, and ability to tolerate oral therapy. Always follow your prescribed formulation exactly—do not substitute between forms without your prescriber’s guidance.

How should Noxafil be taken?
How Noxafil is taken depends on the formulation:

  • Delayed-release tablets: Swallow whole with water. Take with food when possible, but may be taken with or without food. Do not crush, chew, or divide the tablets.
  • Immediate-release oral suspension: Shake well before use. Take during or within 20 minutes of a full meal, an oral liquid nutritional supplement, or an acidic carbonated beverage (such as ginger ale). Food is critical for proper absorption.
  • Delayed-release oral suspension (pediatric): Mixed using the provided kit and administered with food within 1 hour of preparation.
  • Intravenous solution: Administered by a healthcare professional, typically over 90 minutes via central line.

Follow your prescriber’s instructions exactly. If you have trouble swallowing or eating full meals, contact your prescriber—alternative formulations or antifungal therapies may be appropriate.

What are common side effects of Noxafil?
Common side effects of Noxafil (posaconazole) reported in clinical trials include nausea (19% to 38%), diarrhea (18% to 42%), vomiting (12% to 29%), fever (21% to 45%), headache (12% to 28%), abdominal pain (10% to 27%), skin rash (15% to 24%), low potassium levels (22% to 30%), constipation (8% to 21%), cough (9% to 24%), and fatigue (8% to 17%). More serious but less common side effects can include liver problems, irregular heart rhythm (QT prolongation), severe allergic reactions, and pseudoaldosteronism (a condition involving elevated blood pressure and electrolyte changes). Patients should report any unusual symptoms to their prescriber promptly. This is not a complete list of side effects.
What medications interact with Noxafil?
Posaconazole is a strong inhibitor of the CYP3A4 liver enzyme and can significantly increase blood levels of many medications. Coadministration is contraindicated with:

  • Sirolimus
  • Ergot alkaloids (such as ergotamine and dihydroergotamine)
  • HMG-CoA reductase inhibitors (statins) metabolized through CYP3A4, including atorvastatin, lovastatin, and simvastatin
  • CYP3A4 substrates that prolong the QT interval, such as pimozide and quinidine
  • Venetoclax during initiation or ramp-up phase in patients with CLL or SLL

Many other medications require dose adjustments when combined with posaconazole, including tacrolimus, cyclosporine, ibrutinib, vincristine, midazolam, and certain HIV medications. Always provide your prescriber and pharmacist with a complete list of every medication, supplement, and over-the-counter product you take.

Why do I need blood tests while taking Noxafil?
Posaconazole blood levels can vary significantly between patients, especially for those taking the immediate-release oral suspension. Therapeutic drug monitoring (TDM) involves measuring posaconazole levels in your blood to make sure you are getting enough medication to fight or prevent your fungal infection without reaching toxic levels. Your prescriber may order TDM after you have been on therapy for at least 5 to 7 days, and periodically afterward based on your clinical situation. Other routine monitoring may include liver function tests, electrolyte panels (potassium, magnesium, calcium), complete blood count, and kidney function. Can pediatric patients take Noxafil?
Yes. Noxafil is FDA-approved for pediatric patients 2 years and older for the prevention of invasive Aspergillus and Candida infections, and for the treatment of invasive aspergillosis. The delayed-release oral suspension is specifically formulated for pediatric patients ages 2 and older weighing 10 to 40 kg, and the delayed-release tablets are used in pediatric patients weighing more than 40 kg. The immediate-release oral suspension is approved for pediatric patients 13 years and older for oropharyngeal candidiasis. Your child’s prescriber will determine the appropriate formulation and dose.
