8. How does Lupron (leuprolide) work?

Leuprolide is an agonist of gonadotropin releasing hormone (GnRH) receptors. It produces an initial increase in luteinizing hormone (LH) and follicle stimulating hormone (FSH), which leads to a transient increase in testosterone (in males) and estrogen (in females). With continuous administration, leuprolide then results in suppression of ovarian and testicular steroidogenesis due to decreased levels of LH and FSH. In males, testosterone levels are reduced to below castrate levels. Testosterone suppression occurs in approximately 2 to 4 weeks of continued therapy.

  • Eligard: Subcutaneous injection. Vary or rotate injection site; choose site with adequate subcutaneous tissue that does not have excessive pigment, nodules, lesions, or hair. Must be administered within 30 minutes of preparation.
  • Camcevi: Subcutaneous injection in the upper or mid abdominal area.

  • Cardiovascular risk: A higher chance of heart attack, stroke, or sudden heart death has been noted when drugs like this were used in males
  • QT prolongation: Lowering male hormones in the body may raise the chance of a type of heartbeat that is not normal called prolonged QT interval
  • Tumor flare: Symptoms may get worse during the first few weeks after starting treatment. Cases of paralysis and even death have happened in some people with advanced prostate cancer
  • Bone density loss: This drug may cause weak bones, especially with long-term use
  • High blood sugar: New or worsening diabetes may occur
  • Pituitary apoplexy: A rare but serious pituitary gland problem, usually within 2 weeks after the first dose

13. Is there a generic version of Lupron Depot?

Generic leuprolide acetate injection is available for some formulations. However, the long-acting depot formulations require specialized microsphere technology. Do not use a fractional dose of the 3-, 4-, or 6-month depot formulation, or a combination of doses of the monthly depot formulation or any depot formulation due to different release characteristics.

14.What should I tell my doctor before starting Lupron?

  • If you are allergic to this drug, GnRH, GnRH-agonist analogs, or any component of the formulation
  • If you have unexplained vaginal bleeding (females)
  • If you are pregnant, may be pregnant, or breastfeeding — leuprolide is contraindicated in pregnancy and may cause fetal harm
  • If you have bladder blockage or growths on or near the spine or spinal cord (males)
  • History of heart disease, diabetes, or depression
  • All medications you are taking

15. Can Lupron affect fertility?

Leuprolide suppresses ovarian and testicular steroidogenesis. Decreased libido, impotence, erectile dysfunction, and menstrual disorders may result, and fertility may be impaired. However, suppression of fertility is reversible following discontinuation of leuprolide. Pregnancy rates are not expected to be affected once leuprolide is discontinued. Most of the time, this drug stops menstrual bleeding in females — this is not a method of birth control. A nonhormonal contraceptive should be used during leuprolide therapy if contraception is needed.

16. How long do I need to take Lupron?

Treatment duration depends on your condition:

  • Prostate cancer: Treatment is usually continued, including after development of metastatic (castration-resistant) disease
  • Endometriosis: Initial treatment maximum 6 months as monotherapy or with add-back therapy. Total duration of therapy should not exceed 12 months
  • Uterine fibroids: Up to 3 months preoperatively (in combination with iron)
  • Central precocious puberty: Continued until the appropriate age for the onset of puberty; therapy is typically discontinued at approximately 10 to 12 years of age
