Ofev (nintedanib) is an oral prescription medication approved by the FDA for three related lung conditions in adults:

FDA-Approved Indications for Ofev:

  • Idiopathic pulmonary fibrosis (IPF): A chronic, progressive scarring of the lung tissue with no known cause. Ofev slows the rate of decline in lung function in adults with IPF.
  • Progressive pulmonary fibrosis (PPF): Ofev is indicated for chronic progressive fibrosing interstitial lung diseases (ILDs) — a broad category covering ILDs of various causes that continue to worsen despite treatment. This includes ILD associated with conditions such as rheumatoid arthritis, sarcoidosis, and hypersensitivity pneumonitis.
  • Systemic sclerosis–associated interstitial lung disease (SSc-ILD): Ofev is indicated to slow the rate of decline in pulmonary function in patients with ILD associated with systemic sclerosis (scleroderma).

Ofev is not a cure for any of these conditions and does not reverse existing scar tissue — it works by slowing disease progression and preserving lung function over time.

Ofev (nintedanib) is taken orally as a capsule, twice daily approximately 12 hours apart.

Ofev Dosing Information:

  • Standard dose (no liver impairment): 150 mg orally every 12 hours with food. Maximum dose: 300 mg per day.
  • Reduced dose (mild hepatic impairment, Child-Pugh A): 100 mg every 12 hours with food
  • Moderate to severe hepatic impairment: Ofev is not recommended
  • If a dose is missed, skip it and take the next dose at the regularly scheduled time — do not double up

Important Ofev Warnings:

  • Liver toxicity: Serious cases of drug-induced liver injury, including cases with fatal outcomes, have been reported. Liver function tests (ALT, AST, bilirubin) are required before starting Ofev and regularly during the first 3 months of treatment. Risk may be higher in women, patients weighing less than 65 kg, Asian patients, and older patients.
  • Diarrhea: Severe diarrhea can lead to life-threatening dehydration if not managed promptly. Patients should contact their doctor immediately if diarrhea is severe or persistent.
  • Bleeding: Ofev may increase the risk of bleeding, including serious and potentially fatal bleeding events. Use with caution in patients at risk for bleeding or taking anticoagulants.
  • Arterial thromboembolic events: Events including myocardial infarction (heart attack) have been reported. Use with caution in patients with known cardiovascular risk factors.
  • GI perforation: Rare cases of gastrointestinal perforation, some fatal, have been reported. Use with caution in patients with recent abdominal surgery, diverticular disease, or taking NSAIDs or corticosteroids.
  • Fetal harm: Ofev can cause birth defects or fetal death. Patients who can become pregnant must use highly effective contraception during therapy and for at least 3 months after the last dose.

When these ILDs follow a progressive course and fail to respond to standard therapies, Ofev is indicated to slow further decline in lung function.

Patients with PPF are typically managed by a pulmonologist, often in collaboration with a rheumatologist or other specialist depending on the underlying cause.

Systemic sclerosis–associated interstitial lung disease (SSc-ILD) is a lung complication of systemic sclerosis (scleroderma), a rare autoimmune disease that causes hardening and tightening of the skin and connective tissues throughout the body. When systemic sclerosis affects the lungs, it causes scarring and inflammation of the lung tissue (ILD), which progressively impairs breathing and lung function.
